For your convenience, please do not complete either Screening Form or the Reference Check Information Form unless you have been asked to by a member of Crossings staff.. If you are interested in volunteering we have provided this quick Volunteer Interest Form. Completing the form will help us get you started in the right ministry.

Confidential Screening Form
This form is for all volunteers who work with minors, elderly, and disabled. It includes an OSBI Background Check and reference information. Printable PDF form

Confidential Renewal Screening Form
The Confidential Screening Form expires after 3 years and needs to be renewed; this form is for those volunteers who have already completed a Confidential Screening Form and need to renew it. Printable PDF form

Reference Information Form
This form is for those who have not been asked to or required to complete the Confidential Screening Form, and who only need to provide reference information. Printable PDF form
