Volunteer Forms

If you are interested in volunteering at Crossings, completing this quick Volunteer Interest Form will help us get you started in the right place.

Because safety is extremely important to us, volunteers are asked to complete these screening forms after you have identified the area in which you are interested in assisting and have spoken with the Crossings personnel in that area. Please print and complete the appropriate form(s) and return to Elena at 302.1286 (fax) in the Volunteer Ministry office.
(PLEASE NOTE: For your convenience, please do not complete the Confidential Screening Form unless you have been asked to do so by a member of Crossings staff.)

Confidential Screening Form
This form is for all volunteers who work with minors, elderly, and disabled. It includes an OSBI Background Check and reference information.

Confidential Renewal Screening Form
The Confidential Screening Form expires after 3 years and needs to be renewed; this form is for those volunteers who have already completed a Confidential Screening Form and need to renew it.
